Doctor's Charting
A collection of documentation statements actually found on patient's charts during a recent review of medical records. These statements were written by various health care professionals including (we are afraid) a doctor or two at several major hospitals.

The lab test indicated abnormal lover function.
The baby was delivered, the cord clamped and cut, and handed to the pediatrician, who reathed and cried immediately.
Exam of genitalia reveals that he is circus sized.
The skin was moist and dry.
Rectal exam revealed a normal size thyroid.
The patient had waffles for breakfast and anorexia for lnch.
She stated that she had been constipated for most of her life until 1989 when she got a divorce.
Between you and me, we ought to be able to get this lady pregnant.
The patient was in his usual state of good health until his airplane ran out of gas and crashed.
I saw your patient today, who is still under our car for physical therapy.
The patient lives at home with his mother, father, and pet turtle, who is presently enrolled in day care three times a week.
Bleeding started in the rectal area and continued all the way to Los Angeles.
Both breasts are equal and reactive to light and accommodation.
She is numb from her toes down.
Exam of genitalia was completely negative except for the right foot.
While in the emergency room, she was examined, X-rated and sent home.
The patient was to have a bowel resection. However, he took a job as a stockbroker instead.
The patient suffers from occasional, constant, infrequent headaches.
Coming from Detroit, this man has no children.
Examination reveals a well-developed male lying in bed with his family in no distress.
Patient was alert and unresponsive.
When she fainted, her eyes rolled around the room.

Hello everyone!
I hope that you like my blog :)

Today I want to remind you that rest is very important to us. Nursing is a job which takes all our internal power, and for restoration of this power we need a good sleep. I can understand that everyone has family, house holding duties and more, however, I encourage you to think about yourself a little bit more. Imagine you are tired and came to your unit to work. Eight or even twelve hours of non stop running, washing, caring. Can you work effectively if the only thing you want is sit down and close your eyes? I know that everyone have been in this situation. We constantly take minutes and hours from our rest time to do something “more important”.
Stop for a moment. Think about yourself. Your patients do not want a tired nurse, you children do not want a tired mom and your husband does not want a tired wife. Stop for a moment. Cleaning can wait till weekend.

I wish all of you good rest after heavy shift in your units.

Nurses, you have to rest more :)

All of you have to get good sleep like this puppy or kitty.

Hello, Hello, and Hello!!!
My dear friends and future readers, my name is Katya and I am a nurse. Nursing is quite serious profession, isn’t it? That is why I decided to create a blog where people can relax a little bit. So, it is funny blog. I found out that people do not smile too much now a day. Too much work, too much house holding problems, too little time for all these problems. This is the reason why I just want you to smile, at least once, while you will read my blog. All medical professionals support great idea that people who smile and laugh a lot will live longer and happier life.
